Degree Issuance and Procedures

DEGREE ISSUANCE POLICY & PROCEDURE

1. Purpose

To define a clear, consistent, and efficient process for handling degree and transcript issuance for graduates of SZABMU‑affiliated programs, ensuring compliance with SZABMU regulations and administrative transparency.

2. Scope

Applies to all enrolled students of the Institute who have successfully completed their academic programs under SZABMU affiliation and are eligible for award of degree and transcript.

3. Authority

·     Policy: SZABMU has statutory authority to award degrees and transcripts.

·     Institute Role: Midwest Institute facilitates the administrative process for eligible graduates in accordance with SZABMU regulations.

4. Policy Statements

1. Only students of the Institute who have passed all semesters and examinations under SZABMU affiliation and completed all institute clearance requirements are eligible for degree and transcript issuance.

2.  Graduates must complete all required forms, submit necessary documents, and pay fees as prescribed by SZABMU.

3.  The institute ensures proper documentation, verification, and clearance before forwarding applications to SZABMU.

4.   The process will be reviewed periodically to ensure compliance with SZABMU regulations and best practices for affiliated institutes.

5. Procedure for Degree & Transcript Issuance

Step 1: Graduate Clearance

Before recommendation to SZABMU, the student must complete the Institute Graduate Clearance Proforma, obtaining approvals from:

1.     Programme Head – academic completion verified.

2.     Institute Examination Section – final results entered and documented.

3.     Quality Enhancement Cell (QEC) – verification of standards.

4.     Finance Office – clearance of any pending dues.

5.     Admission Office – academic record verification.

6.     IT Office – return of institute IT assets/accounts cleared.

7.     Library – return of books/resources and clearance offines.

8.     The Director of the Institutecountersigns the proforma.

9.     The completed proforma is routed to the Institute Examination Section for record keeping.

Step 2: Submission to SZABMU

1.     Student submits SZABMU Degree Issuance Form and Transcript Form.

2.     Attach supporting documents:

·        Attested academic records (DMCs, certificates) as required.

·        Proof of payment of SZABMU degree and transcript fees.

3. The institute provides a covering letter endorsing the application and forwards it to SZABMU’s Examination Department for processing.

Step 3: Fee Payment

1.      Graduates are responsible for paying the prescribed degree and transcript fees toSZABMU.

2.      Proof of payment must accompany all applications.

Step 4: SZABMU Processing

1.       SZABMU verifies documentation and issues official transcripts and degree certificates.

2.      Degrees are issued according to SZABMU schedule (e.g., convocation or official issuance).

Step 5: Record Maintenance

The Institute maintains:

1.       All completed graduate clearance proformas.

2.       Copies of submitted SZABMU forms and fee payment proofs.

3.       Correspondence with SZABMU regarding issuance.

4.       A register of graduates recommended for degree issuance.

Step 6: Compliance and Review

The degree issuance process will be reviewed periodically to ensure compliance with SZABMU regulations and alignment with best practices for affiliated institutes. Updates from SZABMU policies will be incorporated as required.
